Passport Application Form:
One completed application form DS4085 This form must be completed online, printed and signed by the applicant. The passport office will not accept hand-written DS-4085 applications.
Two letters of authorization. Please select the authorization letter that matches the processing location and print two copies. Both must have original signatures.
Important Note: The Letter of Authorization must be signed by the applicant, and the courier company must be handwritten at the bottom by the applicant.
The courier name should match the company name at the top of the letters of authorization that you print.
(The Letter of Authorization of San Francisco must be completely handwritten with the same color of ink and the same handwriting style.)
Proof Of Departure:
Computer generated flight itinerary from the airline or travel agency, copy of airline ticket or a corporate letter stating U.S. departure by private aircraft or urgent need of passport processing for business travel.
For Rush or Emergency passport service, please call Travisa to make a reservation for your passport appointment.
US Government Fee: $142.00 payment included on the Global Service Order Form or payable in the form of a check made out to the U.S. Department of State.

This is Travisa's service order form, where you will provide your contact and shipping information, choose the passport processing needed and make your payment. You must print the completed form and send it to Travisa with all other supporting documents.
