Passport Name Change Requirements

Passport Requirements:
Valid/signed U.S. passport.

Photo Requirements:
Two passport photos (2x2 with white background). Click here for details.

Passport Application Form:
One completed Reapplication form DS5504 (within one year of passport issuance for name-change).
One completed Renewal Form DS82 (if the passport was issued more than a year ago and less than 15 years).
If the current passport was issued more than 15 years ago, please follow the instructions for New US Passport.

Authorization Letter:
Two signed Letters of Authorization.

Important Note: The Letter of Authorization must be signed by the applicant, and the courier company must be handwritten at the bottom by the applicant. The courier name should match the company name at the top of the letters of authorization that you print.

Proof Of Departure:
Computer generated flight itinerary from the airline or travel agency, copy of airline ticket or a corporate letter stating U.S. departure by private aircraft or urgent need of passport processing for business travel.

Other:
Court-Certified Name Change Decree OR Marriage Certificate (Not a Marriage License) OR Divorce Decree, in the form of a certified copy.

Processing Time:
Passports can be processed as Emergency Same Day or Next Day; Rush 48 Hours or Rush 3-10 Business Days.
For Emergency Same Day/Next Day processing, you must call to Travisa to make a reservation for your passport submission date.

Fee Description:
Complete and print the Global Service Order Form (Order Summary Page) and include all necessary fees.
  • If you are submitting Form DS5504, $60.00 payment included on the Global Service Order Form or payable in the form of a check made out to the U.S. Department of State.
  • If you submitting Form DS82, $170 payment included on the Global Service Order Form or payable in the form of a check made out to the U.S. Department of State.

    Global Service Order Form:
    This is Travisa's service order form, where you will provide your contact and shipping information, choose the passport processing needed and make your payment. You must print the completed form and send it to Travisa with all other supporting documents.
    Mailing Instructions:
    1. Complete and print out all forms.
    2. Check all information for accuracy.
    3. FedEx the Global Service Order Form with all of the items above to Travisa.
