Eligibility Instructions: Passport is renewable if:
- You are able to submit your most recent US passport.
- Your most recent passport was issued in your current name.
- Your most recent passport was issued within the past 15 years.
- Your most recent passport was issued when you were 16 years of age or older.
Note: If your passport is damaged or mutilated, you must follow New Passport application procedures.

Passport Requirements: Most recent U.S. passport. (Issued less than 15 years ago).

Passport Application Form: One completed application form
DSP-82.This form must be completed online. The passport office will not accept hand-written DS-82 applications. This form must have the original signature of the applicant.

Authorization Letter: Two letters of authorization. Please select the authorization letter that matches the processing location and print two copies. Both must have original signatures.

Proof Of Departure: Computer generated flight itinerary from the airline or travel agency, copy of airline ticket or a corporate letter stating U.S. departure by private aircraft or urgent need of passport processing for business travel. This is not required in Chicago for 3-10 day processing.

Other: Please Note: All Passport applications must be two pages printed. The Passport Agency will not accept double-sided applications.

Processing Time: Passports can be processed as Emergency Same Day or Next Day; Rush 48 Hours or Rush 3-10 Business Days.
For Emergency Same Day/Next Day processing, you must call to Travisa to make a reservation for your passport submission date.

Fee Description: US Government Fee: $170.00 payment included on the Global Service Order Form or payable in the form of a check made out to the U.S. Department of State.

Global Service Order Form: This is Travisa's service order form, where you will provide your contact and shipping information, choose the visa processing needed and make your payment. You must print the completed form and send it to Travisa with all other supporting documents.
