The U.S. State Department has formally announced the elimination of visa page inserts for US Passports beginning January 1, 2016. Requests for additional 24-page visa inserts will only be accepted until December 31, 2015. Beginning January 1, 2016, applicants in need of additional pages in their valid passports must renew their passport.

A US Passport is renewable if:

  1. You are able to submit your most recent US passport.
  2. Your most recent passport was issued in your current name.
  3. Your most recent passport was issued within the past 15 years.
  4. Your most recent passport was issued when you were 16 years of age or older.

Note: If your passport is damaged or mutilated, or you do not meet these criteria, you must follow New Passport application procedures.


How to Renew Your Passport:

Proof of Citizenship:

Valid/signed U.S. passport.

Photo Requirements:

Two passport photos (2x2 with white background). Click here for details.

Passport Application Form:

One completed application form DS-82.This form must be completed online, printed and signed by the applicant. The passport office will not accept hand-written DS-82 applications. This form must have the original signature of the applicant.

Authorization Letter:

Two letters of authorization. Please select the authorization letter that matches the processing location and print two copies. Both must have original signatures.

Important Note: The Letter of Authorization must be signed by the applicant, and the courier company must be handwritten at the bottom by the applicant. The courier name should match the company name at the top of the letters of authorization that you print.

Proof Of Departure:

Computer generated flight itinerary from the airline or travel agency, copy of airline ticket or a corporate letter stating U.S. departure by private aircraft or urgent need of passport processing for business travel.

Fee Description:

US Government Fee: $170.00 payment included on the Global Service Order Form or payable in the form of a check made out to the U.S. Department of State.

Global Service Order Form:

This is Travisa's service order form, where you will provide your contact and shipping information, choose the passport processing needed and make your payment. You must print the completed form and send it to Travisa with all other supporting documents.

Mailing Instructions:

  1. Complete and print out all forms.
  2. Check all information for accuracy.
  3. FedEx the Global Service Order Form with all of the items above to Travisa.
