Proof of Citizenship:

Valid, signed U.S. passport.

Photo Requirements:

Two passport photos (2x2 with white background). Click here for passport photo requirements.

Passport Application Form:

One completed Reapplication form DS5504 (within one year of passport issuance for name-change).
One completed Renewal Form DS82 (if the passport was issued more than a year ago and less than 15 years).
If the current passport was issued more than 15 years ago, please follow the instructions for New US Passport.

Authorization Letter:

Two signed forms authorizing Travisa to handle the passport application materials on your behalf. Both forms must have the applicant's original signature. The Letter of Authorization Form is specific to the processing location where you will send your application.

Proof Of Departure:

Computer generated flight itinerary from the airline or travel agency, copy of airline ticket or a corporate letter stating U.S. departure by private aircraft or urgent need of passport processing for business travel.

Other:

Court-Certified Name Change Decree OR Marriage Certificate (Not a Marriage License) OR Divorce Decree, in the form of a certified copy.

Processing Time:

For Rush and Emergency Passport Service call 877-876-3266 to make a reservation so you know what date your application to change the name on your passport can be filed. Travisa will manage this appointment for you, and you will not need to be there in person on this date.

Fee Description:

US Government Fees:
If you are submitting Form DS5504 $60.00 included with your payment to Travisa or payable in the form of a check made out to the U.S. Department of State.
If you are submitting Form DS82 $170 included with your payment to Travisa or payable in the form of a check made out to the U.S. Department of State.
